filling out this form, asks about criminal charges, theres only a spot to list one.i have about 5. should i just list the most recent? its about 3 years old the others were about 8-9 years ago. i dont want to fill this out wrong and get denied

Calling the prison would be the best way to get that answered.

Yeah I would call and just be honest when you get there. I know that where I work (max sec state prison) they let people in who have done 15 yrs in prison to visit all the time so it's pretty hard to get denied

No attach another page listing all your criminal charges. If you do not list them all the prison will deny you visitation.
